Hours of Operation:
Monday - Friday 8:00 a.m. - 5:00p.m.
Cost:
$35.00 Half Day (under 4 hours)
$65.00 Full Day (4 hours and over)

Support Group
A family care giver support group meets every 3rd Wednesday of the month from 10:00-11:00 a.m. at the Carr Center. Discover information about medical issues, dementia, and community resources, while enjoying fellowship with others. In-house care giving for your loved one can be provided at the Center if pre- arranged. Join us for friendship and support! For more information give us a call.

Your doctor may fax a referral for services to our office based off the needs of the potential client and their support system.
Case Manager

We can receive referrals directly from Case Managers with Passport, VA and I/O Waiver.
