How to become part of the family...
Call to schedule a visit with our Director of Nursing to determine if Adult Day Health Services is right for you and your loved one.
Your doctor may fax a referral for services to our office based off the needs of the potential client and their support system.
Case Manager Referral:
We can receive referrals directly from Case Managers with Passport, VA and I/O Waiver.
Hours of Operation:
Monday - Friday 7:00 a.m. - 4:30p.m.
$30.00 Half Day (under 4 hours)
$60.00 Full Day (4 hours and over)
$1.75 per mile

Join our Support Group!
A family care giver support group meets every 3rd Wednesday of the month from 10:00-11:30 a.m. at the Carr Center. Discover information about medical issues, dementia, and community resources, while enjoying fellowship with others. In-house care giving for your loved one can be provided at the Center if pre- arranged. Join us for friendship and support! For more information give us a call.

Powerful Tools for Caregivers program is a 6 week class series that supports caregivers in taking care of yourself while caring for someone else. By taking care of your own health and well-being, you become a better caregiver.
